- Supporters attending the Tottenham fixture must fill in an online health questionnaire - The purpose of the questionnaire is to ensure you do not have any COVID-19 symptoms - It must be completed in order for your ticket to be activated- Please click HERE to access the questionnaire

The questionnaire can be accessed HERE and needs to be completed for your ticket to be activated and to gain access into the stadium.

The purpose of the questionnaire is to ensure that you do not have COVID-19 symptoms (and have not recently been in contact with anyone who has had COVID-19), and that if you have recently travelled outside of the UK, you have complied with any applicable Government quarantine or self-isolation requirements.

If you will be attending the fixture as a member of a group, you must ensure all group members have completed their own individual online questionnaires. If any group member is a minor, you must ensure an adult has completed a questionnaire on their behalf.

If any of your responses to the health questionnaire indicate you should not attend the fixture, the Club will contact you directly to discuss this.
